Japan Renal Biopsy and Kidney Disease Registry (J-RBR/J-KDR)
(Updated on Nov.17.2010)From 2007, the Japanese Society of Nephrology collected renal biopsies on the Japan Renal Biopsy Registry (J-RBR). This is the first nationwide registry of kidney diseases. In 2009, J-RBR was taken over and expanded by the Japan Kidney Disease Registry (J-KDR), which consists of biopsy and non-biopsy cases chronic kidney disease and acute kidney injury including IgA nephropathy, nephrotic syndrome, RPGN, and polycystic kidney disease. The number of cases on J-KDR has reached 7,000 in 2010. A preliminary analysis on epidemiology has been reported, and a second analysis study using J-KDR has commenced, which includes the Japan Nephrotic Syndrome Cohort Study, Japan Incident Dialysis Cohort Study, and Japan IgA nephropathy Study. Researchers can utilize J-KDR for their own analysis studies.

Clinical study for IgG4-related nephropathy
(Updated on Oct.04.2010)IgG4-related disease has been recognized to be a distinct clinical entity. It is characterized by high serum IgG4 concentrations and abundant IgG4-bearing plasma cell infiltration in the involved organs. IgG4-related disease can involve kidney, but the pathological features of the kidney vary from interstitial nephritis to glomerulonephritis. Typical IgG4-related disease involves pancreatitis, but some cases with IgG4-related nephropathy lack pancreatitis. We need to establish a distinct entity of IgG4-related nephropathy and reveal its clinical and pathological features. Japanese Society of Nephrology organized Working Group chaired by Professor Takao Saito, Fukuoka University. They have launched IgG4RN study, which is a retrospective clinical study to reveal clinical-pathological features of IgG4-related nephropathy. Currently, IgG4RNstudy is collecting cases with IgG4-related nephropathy from the members of JSN.

Cadaver transplant in Japan
(Updated on Jul.26.2010)Japan has been suffering from shortage of organ donors for transplantation. JSN and JSDT are working together to promote organ (kidney) donation, in particular to liberate regulation on cadaver transplant.
Recently, MHLW announced the Modification of the Action Guide on Organ Transplantation Law. It has been active since July 17. Major modified points are: 1) age of donor (<6 years): can be done with several conditions, 2) evaluation of brain death: should be filled the diagnostic requirements, and 3) the required documents for organ donation. Basically, this modification will make organ donation possible, if adequately filled -out the papers, even from those without a signed-donor card. However, it may take time that the action change leads to increase in kidney and other organ donation. Oversea transplant should be reduced and is not a choice of treatment as it is banned by international consensus.
